Patient Confidentiality

Please be aware that we are unable to discuss our patient’s medical health with a family member without the patient’s consent.

We respect your right to privacy. Your health record information is only available to Practice staff on a need to know basis, and all information from which the identity of patients could be determined is protected, kept secure and is treated as confidential. All members of The Tollgate Practice team sign a confidentiality agreement.

It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible care.

This information may be used for management and audit purposes. However, it is usually on available to, and used by, those involved in your care. You have the right to know what information we hold about you. If you would like further information about your records, please read our Privacy Notice (GDPR), or contact the admin team.
